Zaid Jabbar, M.D.
Dr. Jabbar has been serving Meadowbrook Manor of Naperville as Medical Director since 2014. Dr. Jabbar's Doctor of Medicine was achieved at the University of Wisconsin, and residency was at Loyola University Medical Center in 2005. Dr. Jabbar's graduate and undergraduate degrees were attained at the University of Wisconsin - Madison. Dr. Jabbar is board certified in Internal Medicine Attending and is the physician leader at Edwards Medical Group. In addition, Dr. Jabbar is the Lead Physician and Lab Director of the Plainfield office, Medical Director of Edward Weight Loss Clinic, and Board Certified Internal Medicine Attending at Hines VA Hospital. Dr. Jabbar's clinical interests include preventative cardiology, hyperlipidemia, and obesity and weight loss.
Shahla Nyaeme, M.D.
Dr. Nyaeme has been serving as the Medical Director of Meadowbrook Manor in LaGrange since October of 2013. Her residency was at John Stroger Hospital in Chicago, and she attended medical school at Al-Mustansiriya University in Baghdad, Iraq. Dr. Nyaeme then achieved her fellowship at Loyola University/VA Hines Hospital in Maywood, IL. Dr. Nyaeme holds board certification in geriatrics and internal medicine, and her specialty is geriatrics. Her hospital affiliations include Adventist Hinsdale Hospital and Adventist La Grange Memorial Hospital. Dr. Nyaeme's practice locations include Adventist Midwest Geriatrics Specialists in Brookfield, La Grange, and Lemont.
Sandeep Kalra, M.D.
Dr. Kalra has been serving as the Medical Director of Meadowbrook in Bolingbrook since 2012. Dr. Kalra's medical residency and internship was in the Internal Medicine department of Ball Memorial Hospital in Muncie, IN. Dr. Kalra's attended medical school at MBBS in Maulana Azad Medical College, Delhi University. Dr. Kalra's experience includes providing geriatric care to long term care facilities, and Dr. Kalra has extensive expertise in geriatric medicine. Dr. Kalra is an active member of the American College of Physicians, and is a past member of the American Medical Association and American Medical Directors Association.
Hazem Al Muradi, M.D.
Dr. Hazem Al Muradi has been consulting for the Cardiology Program at Meadowbrook Manor of Naperville since 2014. Dr. Hazem Al Muradi graduated from the University of Damascus in Syria in 2003. He continued his training in the Department of Medicine at Loyola University for both Internship and Residency and graduated in 2007. From 2007 until 2010, Dr. Al Muradi completed a Cardiovascular Disease Fellowship at Stroger Hospital of Cook County, Illinois. There, he served as Chief Cardiology Fellow. Following Cardiology fellowship, Dr. Al Muradi completed a subspecialty fellowship in interventional cardiology and endovascular intervention at Brown University, Rhode Island. Dr. Al Muradi is board certified in Internal Medicine, Cardiovascular Disease, Interventional Cardiology, General Vascular, Endovascular Medicine, Nuclear Cardiology, Cardiac CTA and Adult Echocardiography. Dr. Al Muradi specializes in both coronary and peripheral vascular interventions. Dr. Al Muradi is affiliated with 6 hospitals, including MetroSouth Medical Center (Blue Island, IL), Edward Hospital (Naperville, IL), Silver Cross Hospital (Joliet, IL), Morris Hospital (Morris, IL), and Provena St. Joseph Medical Center (Joliet, IL).
Farah Hashemi, M.D.
Dr. Farah Hashemi has been serving as the Director of the Wound Care Program in Meadowbrook Manor of Naperville since 2014. Dr. Hashemi oversees all of the wound care patients that have chosen Meadowbrook Manor for their short or long term stays. Dr. Hashemi attended medical school at Northwestern University, Feinberg School of Medicine, and graduated in 1991. Dr. Hashemi had an internship at McGaw Medical Center of Northwestern University, and residency at Ichan School of Medicine at Mount Sinai (Beth Israel), which was completed in 1994. Dr. Hashemi's fellowship hospital was Rush University Medical Center, and this was completed in 1996. Dr. Hashemi is board certified in internal medicine and infectious disease medicine. Dr. Hashemi is currently in practice at Cadence Health Wound Care Clinic and is with Northwestern Medicine Rgional Medical Group in the Wound Care Clinic. Dr. Hashemi is on staff at Cadence Hospital as well as Rush Copley. Dr. Hashemi is fluent in English, Persian, and Spanish.
Jobby Mampilly, M.D.
Dr. Jobby Mampilly has been directing Pulmonary Medical for Meadowbrook Manor in Naperville since 2016. He will see residents previously seen by Dr. Lababidy and will expand upon our efforts to keep residents at their optimal health. Dr. Mampilly graduated from Northwestern University Feinberg School of Medicine in 2005. His residency and fellowship was at Loyola University Medical Center. Dr. Mampilly holds board certification in critical care, internal medicine, and pulmonary disease. Dr. Mampilly was awarded the 2016 Five Star Recipient for Treatment of COPD by Edward-Elmhurst Hospital and currently sees patients out of DuPage Medical Group's Naperville office.
Eric Yang, M.D.
Dr. Yang has been serving as Medical Director of the Wound Care Program for Meadowbrook Manor of Bolingbrook and LaGrange since 2016. Dr. Yang attended Medical School at Northwestern University and graduated in 1987. His residency was in General Surgery at Northwestern University in 1993, and he has been in practice since 1993. Since 2014, Dr. Lang has been the Medical Director of Wound Care Center at AMITA Adventist Medical Center in La Grange. He is a member of AAWC (Association for the Advancement of Wound Care).
Lisa Taylor, APN
Lisa Taylor, APN, is a nurse practitioner with over 10 years of wound care and nursing experience. Lisa attends to patients at both Meadowbrook of Bolingbrook and LaGrange, and she comes to us from AMITA Health Adventist LaGrange Hospital. She specializes in Arterial Insufficency, Chronic Non-Healing Wounds, Diabetes-Like Pressure Ulcers, Foot Wounds, Leg Ulcers, and Venous Ulcers.
Keith Mulki, M.D
Dr. Mulki has been serving as Co-Medical Director of Meadowbrook Manor of Bolingbrook since 2016. Dr. Mulki graduated from the University of Damascus in Syria. Both his Internship and Residency were completed at Christ Hospital and Medical Center. Dr. Mulki is board certified in Internal Medicine and has been in practice since 2005. Hospital affiliation is Silver Cross.
